This group of mission-driven organizations collaborates through CODI to enhance knowledge and support innovation that is only achieved by working together. CODI Partners represent academic medical centers, health and wellness organizations, public health agencies, and social service providers.

North Carolina Coalition to End Homelessness (NCCEH)
The North Carolina Coalition to End Homelessness (NCCEH) is a statewide advocacy organization that upholds human dignity by pursuing justice and engaging innovative partnerships to end homelessness in North Carolina. NCCEH serves as the Homeless Management Information System (HMIS) Lead for 81 counties, providing support to three Continuums of Care and their provider networks to meet Department of Housing and Urban Development reporting requirements. HMIS collects client-level data to help the homeless response system understand who the system serves, the services they receive, and evaluate our effectiveness to equitably end homelessness for all households seeking services.
Chapel Hill Parks and Recreation
The Town of Chapel Hill is nationally regarded for its quality of life. Public parks and recreation are the gateways to a healthier, more livable community. In addition to our parks, trails, indoor and outdoor recreation facilities, Chapel Hill Parks & Recreation offers a myriad of recreation programs and cultural activities for all ages and social backgrounds. Everyone needs the opportunity for self-enrichment, adventure, fellowship and quiet reflection. At Chapel Hill Parks and Recreation Department, we understand these needs and are dedicated to providing cultural and recreational opportunities in a safe and inviting environment.
Durham County Public Health
The Durham County Department of Public Health (DCoDPH) is responsible for public health programs for the entire community. Major services include education, communicable disease control, maternal and child health services, dental health, laboratories, and vital statistics. The Department also regulates sanitation in food-handling establishments, monitors solid waste disposal and conducts disease prevention programs. Regional and local efforts are also being directed towards bioterrorism, disaster preparedness and pandemic response.
Durham Parks and Recreation
Durham Parks and Recreation (DPR) provides opportunities for the Durham community to Play More: Connecting our whole community to wellness, the outdoors, and lifelong learning. Guided by the City of Durham’s Strategic Plan, we strive to help residents discover, explore and enjoy life through creative and challenging recreational choices that contribute to their physical, emotional and social health. DPR is a nationally accredited agency by the National Parks and Recreation Commission for Accreditation of Park and Recreation Agencies (CAPRA).
YMCA of the Triangle
The YMCA of the Triangle provides a vital support system for children, families and adults living in Raleigh, Durham and the broader region. We connect individuals with opportunities to improve their health, make new friends and contribute to a stronger, more cohesive community for all. Offerings consist of adult wellness and fitness programs, after school care, day and resident camps for youth, teen leadership and family development programs. Financial assistance is available.
North Carolina WIC
The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) is a public health nutrition program that is funded by the United States Department of Agriculture. WIC provides access to nutritious foods, nutrition and breastfeeding education, health care referrals to eligible pregnant, breastfeeding, and postpartum women, as well as infants and children. There are 86 WIC Agencies in North Carolina, serving all 100 counties, that provide WIC Program services. These include County health departments, community health centers, and community action agencies located throughout the state.
Duke Health
Duke Health is a world-class academic and health care system that strives to transform medicine and health locally and globally. Duke Health combines Duke University Hospital, Duke Regional Hospital, Duke Raleigh Hospital, the Duke Primary and Specialty clinics, the Duke University School of Medicine, and the Duke University School of Nursing into a network of research, clinical care, and education. The Clinical and Translational Science Institute (CTSI) accelerates real-world translational research at Duke, providing electronic health data for clinical trials, and retrospective analyses and evaluations. Duke CTSI can also assist in identifying Duke Investigators to collaborate on multi-site studies.
UNC Health
UNC Health is composed of 16 Hospitals, 20 Hospital Campuses and more than 500 clinics statewide. Headquartered in Chapel Hill and affiliated with the University of North Carolina School of Medicine, UNC Health is always looking for new ways to care for all North Carolinians. Research at UNC Health includes thousands of physicians and scientists who make basic scientific breakthroughs, engage in translational research that aims to bring lab discoveries into the clinics to benefit the people of North Carolina and the world, and conduct clinical studies with patients.

Duke Clinical Research Institute (DCRI)
As part of the Duke University School of Medicine, DCRI hosts a multidisciplinary team of physician-scientists, researchers, statisticians, and data and informatics specialists with extensive knowledge in clinical trials and observational research. More than three decades of experience and leadership enable the DCRI to help reshape the future of clinical research and medical care. Whether it's transforming the way clinical trials enroll patients, analyzing enormous datasets to understand diseases, developing novel tools for communicating treatment options to patients, or serving as a coordinating hub for data to support related studies, the DCRI is leading the way to a healthier life for everyone.
UNC Collaborative Studies Coordinating Center (UNC-CSCC)
The Collaborative Studies Coordinating Center (CSCC) provides statistical, data management, study management, and quality assurance services for multi-site public health and medical studies. The mission of the CSCC is to improve public health by coordinating important health research, developing innovative research methodology, and providing practical training in the application of research methods. In its 50+ year history, the CSCC has coordinated activities for over 40 large, multi-site clinical trials and epidemiology studies involving hundreds of clinical or field centers and hundreds of thousands of research subjects, and produced over 2,000 research publications.

The National Association of Community Health Centers (NACHC) was founded in 1971 to promote efficient, high quality, comprehensive health care that is accessible, culturally and linguistically competent, community directed, and patient centered for all.
MITRE Corporation
MITRE is the Project Management and Technical Lead for the CODI Project. Sponsored by the Centers for Medicare & Medicaid Services (CMS) on behalf of the Department of Health and Human Services (HHS), MITRE is the operator of the Health Federally Funded Research and Development Center (Health FFRDC) and serves as an objective adviser to all HHS organizations and other federal agencies with health and human services missions. MITRE connects people and data to reinvent health systems, enhance the care experience, and protect the health and well-being of the nation.
