The Data Challenge

Many organizations track and confidentially store information about people, including data about visits to clinicians, demographic information, participation in community programs, and use of social services. But every organization stores and protects data a little differently, which makes collaboration hard. CODI is focused on standardizing, linking, reconciling, and sharing data from participating organizations in a way that protects privacy of individuals and promotes learning for communities.

The CODI-NC Solution

CODI uses existing information technology tools in new ways to link data across clinical and community partners such as WIC North Carolina, Durham and Chapel Hill Parks and Rec, YMCA of the Triangle, The North Carolina Coalition to End Homelessness, Durham County Public Health, and UNC and Duke Health. CODI uses the PCORnet Common Data Model (CDM) to capture information that is unique to CODI such as social determinants of health and program participation from our community-based organizations.

how the linking agent assigns CODI Identifier number

CODI uses an open source technology, called Anonlink, and a data linkage agent to identify unique individuals and create unique person-level identifiers. This is useful because people often appear in more than one data source, This process enables the linkage of records across the CODI network.
